Mazepin was allegedly responsible for the collision after going off-course at Turn 2 before trying to rejoin the race.
He touched with Jack Aitken and then hit Matsushita – and now could be hit with a heavy penalty.
The incident is the latest in a string of high-profile scares in recent weeks.
Earlier in September, Anthoine Hubert tragicallt died after crashing in the Belgian race while Alex Peroni suffered a broken back the following week when his F3 car suddenly took off mid-race.
